Working to improve Type 2 Diabetes outcomes in Western and Far West NSW

This National Diabetes Week 2025 (13-19 July), The Care Partnership – Diabetes (CP-D) program is being recognised for its work improving care for people living with type 2 diabetes across Western and Far West NSW.

A collaboration between the Western NSW Local Health District, the Far West Local Health District, the Western NSW Primary Health Network and RDN, CP-D has already had tangible impacts. 

Five of the 18 individuals being supported by the program have already been credentialed by Australian Diabetes Educator Association and are actively working in rural communities as Credentialled Diabetes Educators. 

RDN’s Health System Manager, Sharif Bagnulo, says the CP-D program is having a powerful impact on growing the Western and Far West NSW workforce.  

“If all 18 health professionals achieve credentialling, this will increase the Western and Far West region’s Credentialled Diabetes Educator workforce by 80 per cent in just three years from a baseline of 23 Credentialled Diabetes Educator’s in 2023, including five who will reach retirement age by 2028,” Sharif said.   

Three of those currently studying are Aboriginal Health Practitioners and are on course to be among the first in NSW to complete the Aboriginal Health Practitioner to Credentialled Diabetes Educator career pathway in NSW and Australia.
